To ensure payments are applied correctly, be sure that each payment has a stub and that each check has a policy number written on the front of check.

• Send payment to the appropriate address above. (Use a pre-addressed envelope if available.)
• Verify that the check has been signed.
• Write the Policy Number on the check.
• Enclose the "Return Stub" (Bottom portion of statement).
• Remember DO NOT , Staple, Paper Clip or tape payments, this will delay payment processing.
